Forms for Parents

If you have children that attend any of the activities provided by NMBC where you are not personally present, we require you to complete consent forms. You will be familiar with this if your child is of school age.

Our standard consent form here for you to download and print off. We need this for our records. We shall also need you to complete an overnight consent form, for use typically when 24/7 have events lasting overnight.
